She was 46. Six months earlier, a urinary tract infection had started it — and when the infection cleared, the blood didn't leave.
For six months, her urine tested 2+ for occult blood. The UTI was gone. There was no urgency, no cloudiness, no pain. Just blood, quietly persisting in the urine, month after month, with no clear cause identified through standard evaluation.
She came in carrying more than the lab result. Varicose veins. Years of constipation — stools dark, sticky, difficult, once every one to two days. Cold hands, cold feet, cold lower abdomen. Poor sleep, many dreams. Headache. Occasional heavy vaginal discharge. Menstrual blood with small clots, accompanied by abdominal and lumbar pain.
Her lower eyelids were pale. Light abdominal tenderness on pressure. Mild scaling on the skin. Faint surface vessels visible beneath the skin.
Every one of these signs pointed to the same underlying condition: a body running low on blood, unable to hold it where it belonged.

The formula prescribed had nothing in it for the kidneys, nothing to stop bleeding, nothing to treat a urinary condition. It was Xiao Jian Zhong Tang — a formula to rebuild the digestive center.
In this case, within the first week, symptoms were reported to have significantly improved. By the end of the second week, the client reported that all symptoms had resolved.
Why Blood in the Urine Is Not Always a Kidney Problem
When a laboratory finds blood in the urine, the clinical investigation understandably focuses on the urinary tract: infection, kidney stones, bladder pathology, glomerulonephritis. This is the appropriate starting point.
When that investigation finds no clear explanation — when the infection has cleared, when the imaging is normal, when the markers don't explain the persistence — conventional medicine and TCM offer different next steps. This case illustrates one TCM approach.
Classical Chinese medicine asks a different question: not where is the blood coming from, but why is the body failing to contain it?

In TCM, blood is not simply a substance that flows through vessels. It is governed — held in its channels, circulated properly, nourished — by the body's central generating capacity. When that capacity is depleted, blood loses its governance. It seeps where it should not.
This woman had every marker of blood deficiency and blood without governance: Pale lower eyelids — the inner surfaces that most directly reflect blood sufficiency. Cold extremities and cold lower abdomen — blood and Yang not reaching the periphery. Varicose veins — blood pooling where it cannot circulate properly. Surface vessels visible beneath the skin — blood struggling to stay contained. Menstrual clots with pain — blood not moving freely. Dark, sticky, difficult stools — the digestive system reflecting the same stagnation and deficiency. Poor sleep, many dreams — the heart insufficiently nourished by blood. Headache — the head insufficiently supplied.
The occult blood in the urine was not identified as a urinary tract problem. It was understood as a governance pattern — blood leaking through the urinary pathway because the body no longer had the resources to hold it properly in its channels everywhere.
The kidneys were not the source, in this assessment. The depleted center was.
Xiao Jian Zhong Tang: Govern Blood by Rebuilding Its Source

Readers of this series will recognize the formula: Xiao Jian Zhong Tang — Minor Construct-the-Middle Decoction. The same formula used for the child with eczema, the woman with allergic purpura, the man with false heat and dry skin. Different presentations. The same root.
Each of those cases involved blood or fluid failing to nourish and contain itself properly — showing up differently at the surface of the body, but arising from the same depletion at the center. In the eczema case, the depletion showed on the skin. In the purpura case, it showed as skin bleeding. Here, it showed in the urine.
Maltose — replenishes the depleted fluids and blood at the center. White peony (double dose) — nourishes blood directly, restrains what is leaking. Cinnamon twig — warms and moves, circulates the replenishment outward. Ginger, jujube, licorice — rebuild the generating foundation.
The formula does not address the urine. It addresses the body that the urine is coming from. When the center is rebuilt and blood is properly governed again, it stops leaking — not because anything has plugged the leak, but because the condition that was causing the leaking has been resolved.
A Pattern Worth Recognizing
This case is worth noting for anyone who has been told that their occult blood — or any persistent low-level bleeding that investigations cannot fully explain — is something to monitor rather than treat.
In Classical Chinese medicine, unexplained persistent bleeding is not a mystery. It is a pattern. And the pattern almost always involves some combination of blood deficiency, blood stagnation, and a depleted center that can no longer govern circulation properly.
The co-occurring symptoms are the evidence: cold extremities, varicose veins, constipation, pale eyelids, poor sleep. These are not incidental findings alongside the urinary blood. They are the same condition, showing up in different places.
When all of these symptoms improved together — as they did in this case — it suggests they shared a common root. And that root was addressable within this TCM framework.

尿潜血半年——一张补脾胃的方子的调理思路

她四十六岁。半年前因尿路感染就诊,感染治好了,尿里的血没有退干净。
此后六个月,尿常规潜血持续2+。没有尿频尿急,没有尿浑浊,没有疼痛。只是血,安静地、月复一月地留在那里,常规检查未能找到明确的原因。
从中医看,就不只看一个报告。静脉曲张。便秘多年,大便色黑、黏滞难解,一两天一次。四肢凉,小腹凉。眠差梦多。头痛。有时白带偏多。月经有血块,伴腰腹痛。下眼睑淡白。腹部轻压痛。皮肤轻微甲错,轻微血络浮露。

中医问的是另一个问题:不是血从哪里来,而是身体为什么无力把血留在它该待的地方?
中医的血,不只是一种流动的物质,它需要被统摄——被固守在经脉里、被正常循环、被充分濡养——而这种统摄能力,来自身体的中焦生化之根。中焦亏虚,血失统摄,就会从不该出的地方渗出来。
下眼睑淡白——最直接反映血液充盈程度的部位。四肢凉、小腹凉——血和阳气无力温达四末。静脉曲张——血液在无法正常循行的地方淤积。皮下血络浮露——血液难以被正常收纳。月经血块伴疼痛——血行不畅。大便色黑黏滞——消化系统呈现同样的淤滞与亏虚。眠差梦多——心血不足,心神失养。头痛——头部血液供养不足。
尿里的潜血,在这个案例的判断中,不是泌尿道的问题,而是统摄失调的问题——身体已经没有足够的力量,把血液完整地固守在全身所有经脉里,尿道这一路就出现了渗漏。
在这一判断中,问题不在肾,在亏虚的中焦。
小建中汤:从源头统摄血液
读过这个系列的人对这张方子应该不陌生:小建中汤。女童湿疹案用过,过敏性紫癜案用过,虚热津血不足案用过。每次表现不同,根是同一个。
湿疹那次,亏虚显现在皮肤。紫癜那次,血渗出在皮下。这一次,血渗出在尿里。来处各异,病机同源——血液失去了中焦的统摄,从最薄弱的地方漏出来。
饴糖——从中焦补充亏虚的津血。白芍(重用)——直接养血,收敛固守漏出的部分。桂枝——温通经脉,使补益到达所需之处。生姜、大枣、甘草——重建中焦生化之根。
这张方子不处理尿,处理的是那个尿从中来的身体。当中焦重建,血液重新得到统摄,渗漏自然停止——不是因为堵住了漏口,而是因为导致漏出的那个根本状态已经被解决了。
一个值得识别的模式
