The Original Case Record
A woman had suffered from diarrhea for several years. She had difficulty eating and could barely rise from her bed. Her body was emaciated and weak; her skin had become scaly and rough. Previous physicians had treated her with Ginseng, Aconite, Terminalia, and Poppy husk — without effect. She came to Shouren Clinic, where Dr. Sun diagnosed her and said: This is the mechanism described in the Jin Gui Yao Lue — When the manifestation is of Yin, rescue it with Yang methods. According to this case record, he prescribed Guizhi Tang, strictly instructed her to induce sweating — and the diarrhea was reported to have stopped. He then continued with Baihe Zhimu Tang combined with millet gruel, and she gradually recovered.
Why Had Previous Treatment Failed?

The previous physicians used Ginseng, Aconite, Terminalia, and Poppy husk — all aimed at stopping diarrhea: tonifying Qi, warming Yang, astringing the intestines. The logic was sound. Yet the patient continued to waste away. From a TCM perspective, this treatment was addressing the manifestation rather than the root — the real source of the problem was not the intestines, but the body's surface defense system (Ying and Wei).
The Diagnosis: Rescue Yin Manifestation with Yang Methods
The analogy of the house: Imagine a building where the basement keeps flooding. Workers keep plugging drains — but the water keeps rising. An old engineer finally says: You have been plugging the wrong place. The water is coming in through the roof. Addressing the roof may help with the underlying issue. This woman's situation was exactly the same. Dr. Sun recognized that the root issue was the body's surface defense system, not the intestines.
Why Does Guizhi Tang Treat Chronic Diarrhea?
The body's Wei Qi system governs the surface — but the same energy system also governs the permeability and tone of the intestinal tract. When Wei Qi is chronically disharmonized, its holding function fails not just at the surface (spontaneous sweating) but also in the middle and lower Jiao — leading to chronic diarrhea. Years of diarrhea had also exhausted blood and Ying Qi, associated with the characteristic scaly, rough skin — a classic sign of Ying blood deficiency.
The key detail — cover and induce sweating: When Wei Qi is activated through gentle perspiration, the interior intestinal regulation may also be supported. As TCM says: When the surface Qi flows, the interior Qi harmonizes naturally.
Step Two: Baihe Zhimu Tang and Millet Gruel

According to this case record, once symptoms had subsided, the[C5] body still needed extensive repair. Years of loss had depleted Qi, blood, body fluids, and organ nourishment. Baihe Zhimu Tang nourishes Yin, clears heat, and moistens body fluids — appropriate after the warm-activating Guizhi Tang phase. Millet gruel gently nourishes the spleen and stomach. This reflects the completeness of TCM treatment: first resolve the primary conflict, then conduct follow-up repair.

一、病案原文
某妇人下利数年,吃饭困难,起卧艰难,形体消癎虚弱,肌肤甲错,前医以人参附子诃子罂粟治之,未曾见效。赴守仁斑,老孙诊之曰:此乃金匮要略百合篇所谓见于阴者,以阳法拯之之机也。据本案记录,与桂枝汤,严嗅取汗,遂下利止。继以百合知母汤,复合小米调之,渐渐复原。
为什么前医无效?
前医用了人参、附子、诃子、罂粟壳——都在止泻:补气固脱、温阳散寒、殁肠止泻。逐辑上无懈可击。然而病人依然形销骨立、无法起卧。说明这条思路走不通——都在堡地下室,而屋顶的缝一直没修。
诊断的转折:见于阴者,以阳法拯之
比喻:想象一幢房子,地下室持续漏水。工人们一直在堡地下室,但水还是越来越多。来了一位老工程师,说:你们堡错地方了。这水不是从地下滲上来的,是从屋顶进来的。屋顶有缝,雨水顺着墙壁流下来,最后积在地下室。这位妇人的情况,正是如此。
为什么腹泻要用桂枝汤?
卫气运行于体表,负责防御外邪和调控汗孔,而负责肠道固摄的气机,根源上是同一套系统的不同表现。当营卫长期失调,卫气无力固摄,不仅表现在体表(自汗),也波及到中焦、下焦导致慢性腹泻。更进一步,数年下利必然耗尽气血,营气大虚,无力滋养肌肤,才会出现肌肤甲错。
覆而取汗这个细节至关重要:当卫气通过发汗得到宣通,表层的气机活了,对里层肠道的调控也随之恢复。用中医的话说:表气通,里气自和。
第二步:白合知母汤的善后智慧
腹泻止住之后,病还没有完全好。数年的消耗,身体已经极度亨虚。白合知母汤养阴清热、润养津液,在桂枝汤温通止泻后,换用滋养、平和的药物来修复损耗的阴液。小米温和,擅养脾胃。这体现了中医治疗的完整性:先解决主要矛盾,再进行善后调理。
