Article 7: The Man in a Winter Coat in Summer — Chills and Spontaneous Sweating

Introduction: An Unusual Summer Scene

It was the height of summer heat when a patient walked into the clinic. The doctor and waiting patients all froze — this man in his fifties was wearing a thick, padded cotton jacket and trousers.

Presentation: The Body's Thermostat Had Failed

The patient's condition: extreme sensitivity to wind and cold even in the height of summer; incessant cold sweating soaking his clothing; a vicious cycle — the more he sweated, the colder he felt; inability to remove the coat (the moment he did, cold sweat broke out all over, cold penetrating to the bone); weak constitution — poor appetite, fatigue, shortness of breath.

TCM Analysis: Disharmony of Ying and Wei

In TCM, the body has two protective systems: Wei Qi (Defensive Energy) — circulating at the surface, governing defense against pathogens and regulating pore opening and closing; and Ying Qi (Nutritive Energy) — circulating within the blood vessels, nourishing all organs. When Wei Qi is deficient, the pores cannot be held closed — like a broken faucet that cannot be turned off. When Ying Qi is insufficient to warm and nourish, and Wei Qi cannot defend against cold, the result is internal and external cold simultaneously.

Treatment and Results: Guizhi Tang

Diagnosis: deficient Zheng Qi, disharmony of Ying and Wei. Prescription: Guizhi Tang.

According to this case record, after 5 doses: chills reduced; the patient was able to ride a bicycle; switched from padded coat to a lighter jacket; sweating decreased. After approximately half a month: wearing a single layer, no longer sensitive to wind, no spontaneous sweating.

Key insight — Bidirectional regulation: Guizhi Tang is not simply a diaphoretic or sweat-stopping formula. It restores the body's own self-regulation: allowing it to sweat when it should and stop when it should.
